Appointments
We know you have many choices when choosing a Chiropractor so we have made requesting an appointment a simple process via our website. If for any reason you cannot keep a scheduled appointment, or will be delayed, please call us as soon as possible.
Some insurance companies require additional paperwork for your initial consultation. For your convenience we have provided these forms for you to download and complete before your appointment. All forms are under the "request an appointment" page.
Insurance
Some of the insurance plans which Dr. Gottesfeld is a preferred provider with are: Atnea, Cigna, United Health Care, Blue Cross/Blue Shield, Anthem, Guardian, plus many others. Our office accepts a variety of HMOs, PPOs, and other health plans. Please call our office to verify acceptance of your insurance carrier.
Our office no longer accepts medicare patients.
